Myrna Lee Forman, of Mountaindale, passed away on Friday, February 15, 2019 at Roscoe Regional Nursing Home. She was 81. She was the daughter of the late Leon L. Forman and Sarah G. Schfrach, born on July 22, 1937 in Brooklyn, NY. A service will be held at 11:30am on Sunday, February 17, 2019 at Yeshiva Zichron Mayir, Mountaindale, NY.
